This page is for use by DNA Chore Service LLC Liaison only. This page is locked to all visitors. Liaisons please use this page for all referrals. You will find useful information about sign up process for new clients and transferring clients below. You now have access to download blank 4676, Medical needs and Adult Service Application. Please let me know if folders are needed and remember to follow text message updates and fb reminders to communicate with your clients.
Please take time and review the sign-up process for all clients. The flow is as follow NO exceptions:
1. All new client must have a new client notification filled out by you soon as you get the info. So I am aware and can get the file started.
2. All caregiver will fill out online application only.
3. An email will be sent for the caregiver to complete payroll and tax withholdings after the online app is completed.
4. Paperwork will NOT be turned in to DHHS without me having copies and reviewing for completeness.
5. The client/caregiver must be given either a black colored folder (new) or a gray colored folder (transfer) and sign that they have received the DHHS Rights and Responsibility, online app link, will complete Square payroll account, and have Agency Contact Sheet. (You should NOT be keeping these folders).
6. Communication is a must from caregivers and clients and it starts from intro. When you visit to sign up a client, it should be a 20 to 30 minute process:(see new client) or (transfer client) for details of each process.
The goal is to have you get all the info you need in one visit. So the only revisit is to pick up a Medical Needs Form.
Also with the new billing coming up, the caregivers who say they can not work a computer or phone will have to learn or resign. The State is requiring time tracking and the caregivers will have to clock in and out while at the clients home. This will take effect in June so I will start getting some online and in person in-services set up starting in May.
A. New client announcement or Online Referral should have been done before you visit the client.
B. For new clients. Go over the Welcome letter with the client and caregiver. Next give medical needs form and let client know to give doctor fax form so the doctor can fax the form or the form can be picked. Then complete the Adult Service Application. Take the Adult Service Application to Cherrylawn to make copies. Once Medical Needs is completed make a copy and take to Cherrylawn for copy and review. Then both forms can go to DHHS Office.

For transferring clients. Review the notification of transfer form and and make sure the steps are followed. Next complete the 4676 Statement of employment. Have the caregiver fill out online application and I will associate them in CHAMPS ASAP.
FYI: ALL TRANSFER CAREGIVERS NEED TO BE ASSOCIATED WITH THE AGENCY BEFORE THE START OF CARE DATE TO PREVENT LOSS OF SERVICE DAYS.
