To receive services, the care recipient must be a Michigan resident, be enrolled in Michigan Medicaid, and have a documented physical, mental, or cognitive condition that limits their ability to perform Activities of Daily Living.
Yes. Adult children, siblings, grandparents, and friends can be paid caregivers. However, legally responsible relatives (like spouses and parents of minor children) are not eligible to be paid under this program.
Awarded hours are not standard and depend on a home visit assessment conducted by your local Michigan Department of Health and Human Services (MDHHS) caseworker. The caseworker uses a rubric to determine how much help is needed with tasks like bathing, dressing, meal prep, and light housekeeping, with most participants qualifying for up to 180 hours of care per month.
As of 2026, the individual caregiver rate in Michigan is $17.13 per hour, which includes a base rate and a state-mandated direct care worker pass-through.
Before an assessment can happen, your doctor or licensed medical provider must complete and sign the DHS-54A Medical Needs form. You will also need to submit proof of Medicaid coverage and identification during the application process.
You must apply for services directly through MDHHS or via the MI Bridges Portal. Once approved, you must register as a provider in the CHAMPS Portal (Community Health Automated Medicaid Processing System) to receive payments.
Caregivers are required to submit their hours weekly using the MI Time system. This system utilizes Electronic Visit Verification (EVV), meaning caregivers must clock in and out using an approved method (usually a mobile app) at the care recipient's home to validate service delivery and secure payment.
