This Contact Sheet must be given to your worker at the start of care or transfer face to face visit with your worker. This will make your worker aware you are working with an Agency and to include the Agency in all correspondences. 

If you are a Live-in Caregiver, you must complete this form and provide a second proof of residency document from the list provided. This form must be turned in and approved by your worker. If not your caregiver will have to clock-in when providing care through the HHAeXchange App.
